- PROGRAM SERVICES. Conducts developmental assessments and evaluations of assigned children, including data collection. Works cooperatively with family members and program team to establish goals and objectives. Implements goals and objectives in natural environments. Schedules therapy sessions to accommodate needs of parents/family. Regularly provides information regarding client progress/service effectiveness to program team and health professionals, including attending goal reviews and Individual Family Service Plan (IFSP) meetings.
- PROGRAM SUPPORT. Observes, documents, and participates in activities with children in Early Intervention Program and performs annual evaluations. Assists with supervising aides, students, paraprofessionals, and volunteers as requested. Assists with maintenance of program supplies/equipment and notifies supervisor of needed supplies/equipment. Provides coaching, consultation, modeling and enrichment to parents/families and program staff to assist with achievement of established goals and objectives.
- ASSISTIVE TECHNOLOGY. Assists children in acquiring devices and adaptations to overcome barriers impeding their progress and development. Designs and fabricates or orders adaptive equipment and devices for home use.
Designs and fabricates modifications to program and home environments as needed, within scope of practice and allowable activities under their license.
- REFERRAL SERVICES. Notifies Service Coordinator of needs for referral to other programs. Serves on interagency committees as requested.
- RECORDS. Maintains progress notes and other required documentation. Provides information on client progress to families on a regular basis, at a minimum at the six-month review and annual assessment. Provides written reports to program team prior to IFSP meetings.
- SAFETY. Assures the safety of clients, families, and staff in work environments.
- POLICIES/REGULATIONS. Maintains knowledge of MDS policies, regulatory requirements and licensing standards and assists in assuring program compliance.
- INSERVICE. Participates in the development of in-service training modules and facilitates training as needed. Provides training for appropriate community organizations and professionals, as requested.

- Current Colorado State License from Colorado Department of Regulatory Agencies (DORA)
- Two years' work experience with special needs children under the age of 3 preferred.
- Satisfactory results from CBI screening and reference checks per guidelines.
- Valid driver's license, insurable and personal vehicle available for work-related travel.
- Ability to lift, transfer or carry 25 lbs. to 40 lbs. infrequently.
- Ability to stand/walk at least 20 minutes per hour.
- Ability to push/pull 40 lbs. frequently, up to l00 lbs. infrequently.
- Ability to reach, stoop, kneel, squat and crawl frequently.
- Ability to work with hands and manipulate fingers.
- Ability to respond to emergencies, as needed.
- Ability to operate office equipment including, but not limited to, telephone, computer, fax and copy machine.
- Ability to visit child's residences and other community locations.

Description
POSITION OVERVIEW:
The Early Intervention Program serves children 0-3 years old who are developmentally delayed or disabled and provides developmental programs and support services to children and their families.
The Therapist provides assessments, evaluations and therapeutic intervention for children enrolled in the program in conjunction with parents/families, program teams, other staff, health professionals and outside agencies.

Requirements
Job Functions
1.

Works closely with Assistive Tech Consultant.4.
DOCUMENTATION and BILLING. Assures child's information has been placed into the proper data systems to allow for timely and effective billing practices. Maintains progress notes and other required documentation.

TRAINING
Complete the following:
a. Early Intervention Provider Training course, which includes 13 modules and a final exam.
b. Training for Telehealth Service Delivery
c. Successful completion of assigned STRIVE CORE Classes.
d. Successful completion of CPR for children

SAFETY. Assures the safety of clients, families, and staff in work environments.
8.
POLICIES/REGULATIONS. Maintains knowledge of STRiVE policies, regulatory requirements and licensing standards and assists in assuring program compliance.
9.
INSERVICE. Participates in the development of in-service training modules and facilitates training as needed. Provides training for appropriate community organizations and professionals, as requested.
