Description
Care Advantage Skilled a Care Advantage Inc company, (Formally known as All About Care) Voted #1 in Home Healthcare the last 5 years is looking for a Clinical Manager/RN to join our skilled nursing team in a Full-Time capacity.
Our team provides 1 on 1 care in patients' homes with extensive services to provide our patients with the best possible care.
The Clinical Manager is responsible for monitoring, coordinating, and managing all aspects of patient clinical care administered by the field clinicians.
The Clinical Manager will effectively interact with patients, caregivers, family members and provide leadership to field clinicians to ensure quality care on all levels.
Minimum of 3 years Home Health experience - REQUIRED.Previous experience with Homecare Homebase EMR system - PREFERRED.
Minimum of Bachelor's degree, or LPN with five years of experience
Ability to apply and analyze data as it pertains to quality and compliance metrics.
