- Assess patient's needs comprehensively
- Create a care plan in collaboration with the patient, family, and health team
- Implement and supervise the care plan
- Evaluate patient's response to care and adjust the plan accordingly
- Document patient care accurately and timely
- Participate in research and educational activities
- Contribute to improving patient outcomes and national nursing quality indicators
- May involve supervising staff and department workflow
- Interact effectively with healthcare staff, patients, families, and external contacts
- Provide spiritual support when needed
- Must have a valid RN license
- BLS certification is mandatory
- Experience in the relevant clinical area is preferred
- Home Health team members must have a valid driver's license and auto insurance
- Specific facility requirements may apply
- Bachelor's degree is a plus
- Professional certification is preferred
- Additional related education and experience is a bonus
