About the Position: Summary:
Role
Under minimal supervision the Staff Physician provides medical leadership to the medical team and is responsible for the provision of medical care within the physician's specialty, scope of practice and area of expertise to the Community Health Center of Yavapai (CHCY) patients in an outpatient setting.
Major Duties, Responsibilities
Provides comprehensive medical care in accordance with regulations and scope of practice for licensure in Arizona, State, Federal and other grant requirements and the requirements of all payors. Medical care will include assessment, diagnosis, planning of treatment and evaluation of therapy. Staff physician will examine patients and conduct appropriate tests to determine medical condition/health status, determine and implement treatment plans and/or refer patients to specialists and other medical facilities as needed. Maintains 36 hours of patient appointments per week and a minimum of 40 hours of work. Complies with the Provider Appointment Schedules Policy CLS#114 as to number of appointments in schedule and patients seen on a daily basis. Supervises medical team, including review and co-signing of patient record entries by mid-level providers, as designated by policy and procedure. Documentation of all medical services and patient interactions will be entered into the Electronic Health Record in accordance with department standards, all federal reporting requirements, (Uniform Data System), Meaningful Use criteria, Patient Centered Medical Home and standards of the quarterly Quality Improvement audits including in Chronic Medical Conditions such as Diabetes (DM) and Hypertension (HTN). All documentation must be complete and include a physical examination and review of systems on every patient. Documents according to Department standards. Provides leadership and direction in the improvement of one of the Clinical Performance Measures and participates in the other Quality Improvement Program initiatives, as requested. Participates in on call duty, as required. Represents the CHCY in community outreach events such as health screenings/physicals at Health Fairs, school sports physicals and community sports physicals, as assigned.

Expectations:
Minimum Qualifications
Education:
Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.) degree from an accredited medical school, and at least three years of residency/internship experience.
Active, valid Arizona license and federal DEA license required.
Board certification in area of specialty or eligibility to obtain certification within one year of hire. BLS required.
Experience:
One year experience providing patient care in a clinical setting, preferred. Experience with using electronic health record, preferred.

Interpersonal Skills:
Provides medical care focusing on serving the patient. Practices clear and tactful communication with all patients. Provides excellent customer service by demonstrating a helpful, approachable attitude, attention to detail and follow-through. Understands and demonstrates a high level of customer service, including the confidential handling of proprietary information.
Other Skills:
Computer competent. Uses the electronic medical record system for medical record management. Valid Arizona State driver's license, without restrictions. Willingness to travel to other CHCY locations within Yavapai County.
