    Description
    The Housing Navigator role provides housing service navigation activities, including monitoring, referral, application for entitlements, illness education and counseling to support individuals with serious mental illness in maintaining stable community living. The navigator works closely with the treatment team to assess an individual's needs and develop personalized plans that address these needs. They coordinate referrals and linkages to relevant medical behavioral social programs services supports as well as promote natural Supports through planning collaborative work sessions. Additionally they conduct monthly home visits assessing progress towards stabilized supportive living goals and document all service delivery accurately following policies procedures regulations.
