    Job Title: Nurse Practitioner

    Description:

    Nurse Practitioner Qualifications:

    • 2 years of experience as a Nurse Practitioner in acute care environments.
    • Masters of Science in Nursing - Family Nurse Practitioner degree.
    • Active, unencumbered state NP license.
    • Current driver's license and auto liability coverage.
    • Detailed assessment skills for Hospice patients require recertification.
    • Interest in community-based roles requiring travel between patient visits.
    • Drug Enforcement Agency (DEA) number required.
    • National Provider Identifier (NPI) required.

    Responsibilities:

    You will work collaboratively with our team to provide high-quality patient care. This includes:

    • Delivering personalized care plans tailored to each patient's needs.
    • Collaborating with interdisciplinary teams to ensure comprehensive care.
    • Communicating effectively with patients, families, and healthcare professionals.
    • Fostering a culture of trust, respect, and compassion within our organization.
