- Provide home visits to pregnant clients and their families eligible for the NFP Program, with a caseload of up to 13 families.
- Adhere to nursing process and the NFP model of home visitation.
- Maintain long-term therapeutic relationships with families.
- Provide education to families around physical and mental health, pregnancy, parenting, child development, safety, healthy relationships and many more topics.
- Use motivational interviewing to identify client priorities and concerns and support clients to accomplish goals.
- Connect families to community services and advocate for families within services and systems.
- Perform care coordination with other service or health care providers when indicated.
- Complete timely documentation of home visits according to agency policy.
- Participate in weekly team meetings and case conferences.
- Participate in weekly reflective supervision sessions with supervisor.

Summary
Whatcom County Health and Community Services (WCHCS) is recruiting for a 0.50 FTE Public Health Nurse Job Share position for our Nurse-Family Partnership team (NFP). NFP is an evidence-based home visiting program that supports the health and well-being of young families through a relationship-based approach.
The NFP Nurse Home Visitor is responsible for providing comprehensive nursing services to pregnant individuals and their families eligible for the NFP Program. The Nurse Home Visitor (NHV) is pivotal in transforming the lives of vulnerable babies and parents by implementing and maintaining fidelity of the NFP model. We center our work in health equity with our aim being that all children are healthy, families thrive, and communities prosper. Qualifications
Requires a Bachelor of Science Degree (or higher) in Nursing with coursework in public health AND a current, valid Washington State license to practice as a Registered Nurse.
Education requirement can be met with current enrollment in a Bachelor or Master of Science in Nursing program, with graduation within six months of hire.
Two years recent experience in parent/child health, public health, home visiting or mental/behavioral health nursing preferred.
Demonstrated fluency in Spanish strongly preferred.

Written and Verbal Communication- NHVs need to communicate effectively, openly and consistently with their families, colleagues and stakeholders.
Nursing Knowledge: Strong assessment skills to identify when medical care for a client, child or family member may be indicated and/or communication with medical team is warranted.
Time Management- Ability to self-manage a flexible and dynamic schedule while meeting the needs of families.
Ability to impose structure- in between and during home visits, NHVs must be able to create structure in moments of confusion, disorganization or crisis.
Self-reflection- Successful NHVs actively work to understand and address their person reactions to clients, welcome feedback non-defensively, and make good use of the reflective supervision provided.
