Job summary
The Consultative & Infusion Pharmacist serves as a hybrid clinical and sterile-compounding specialist supporting hospice patients. This role combines responsibilities as a clinical consultant to hospice care teams and as an infusion/sterile compounding pharmacist.Responsibilities
- Provide expert hospice medication therapy consultation, including symptom-management recommendations, deprescribing guidance, formulary support, and medication selection aligned with goals of care.
- Evaluate drug therapy plans, document clinical findings, and communicate recommendations to prescribers,,hospital IV room required.
