    Description
    We are seeking a dedicated and experienced Registered Nurse (RN) to join our clients Assertive Community Treatment (ACT) team.

    As an integral member of their multidisciplinary team, the ACT Team RN will provide comprehensive nursing services to individuals with severe and persistent mental illness who require intensive community-based support.

    The ideal candidate will have a minimum of 2 years of experience in the behavioral health field and a passion for delivering high-quality, person-centered care to individuals in need.


    Responsibilities:

    • Conduct comprehensive assessments of clients' physical and mental health needs, including medication management, psychiatric symptoms, and social determinants of health.
    • Collaborate with psychiatrists, therapists, case managers, and other team members to develop individualized care plans based on clients' strengths, preferences, and treatment goals.
    • Coordinate and monitor the implementation of care plans, ensuring that clients receive timely access to appropriate services and interventions to support their recovery and community integration.
    • Administer psychiatric medications and provide medication education, monitoring, and adherence support to clients in accordance with established protocols and nursing standards.
    • Educate clients and their families/caregivers about the effects, side effects, and risks of medications, as well as strategies for managing medication-related issues and promoting overall health and wellness.
    • Monitor clients' physical health status, including vital signs, lab results, and symptoms of medical conditions, and collaborate with healthcare providers to address any medical concerns or emergencies.
    • Respond to crisis situations and provide immediate nursing interventions, including de-escalation techniques, assessment of safety risks, and coordination of emergency services as needed.
    • Provide emotional support, counseling, and coping skills training to clients and their families/caregivers during times of crisis or distress, promoting resilience and crisis stabilization.
    • Collaborate with community resources, emergency services, and psychiatric facilities to ensure seamless transitions of care and continuity of services for clients in crisis.
    • Maintain accurate and timely documentation of nursing assessments, interventions, and client progress in electronic health records (EHR) and other clinical documentation systems.
    • Prepare and submit reports, progress notes, and other documentation required for program compliance, quality improvement initiatives, and billing purposes in accordance with agency policies and regulatory requirements.
    • Participate actively in team meetings, case conferences, and clinical supervision sessions to review client cases, discuss treatment strategies, and coordinate care planning efforts.
    • Seek opportunities for professional development and continuing education to enhance clinical skills, stay abreast of best practices in psychiatric nursing, and contribute to the ongoing improvement of ACT team services.
    • Foster a positive and supportive team culture, promoting open communication, collaboration, and mutual respect among team members to achieve shared goals and objectives.

    Requirements:

    • Registered Nurse (RN) licensure in the state of practice.
    • Minimum of 2 years of experience in the behavioral health field, preferably in a community-based setting or working with individuals with severe and persistent mental illness.
    • Strong assessment, clinical judgment, and crisis intervention skills, with the ability to respond effectively to complex and challenging situations.
    • Knowledge of psychiatric medications, psychotropic side effects, and principles of medication management in mental health treatment.
    _Please note- The annual base salary range is $90k to $110k.

    Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things.
