    As we expand our residential services to support more adults in achieving their personal goals, we're creating an exciting opportunity for a Case Manager who wants to make a lasting difference. You'll help shape individual support plans and connect participants with the resources they need to live as independently as possible.
    About This Opportunity
    In this role, you'll serve as the primary advocate and coordinator for approximately 12 participants, working directly with our Case Management Supervisor to design person-centered support plans that reflect what each individual wants to achieve. Based at our Libertyville campus, you'll collaborate with direct care staff, families, and community partners while enjoying meaningful work that creates visible impact every day.
    What you'll accomplish:
    • Design individualized support plans that help participants achieve personal goals in work, health, and community life
    • Lead collaborative planning meetings that bring together families, guardians, and service providers around each person's vision
    • Build partnerships with healthcare providers, therapists, and community organizations to expand participant opportunities
    • Train and guide direct care staff on implementing support strategies that respect individual choices
    What you bring to this role:
    You bring experience working with adults with developmental disabilities and understand how to balance individual choice with safety and compliance. Your ability to build trust with participants and families, combined with strong organizational skills, makes you effective at coordinating complex support services.
    • Bachelor's degree in social work, psychology, healthcare, or related field, plus one year working with people with developmental disabilities
    • Qualified Intellectual Disabilities Professional (QIDP) certification or ability to complete within six months
    • Valid driver's license from Indiana, Illinois, or Wisconsin with clean driving record
    • Strong written and verbal communication skills and proficiency with documentation software
