    An Intake assessment Nurse LVN is a LVN with home health experience whose primary responsibility is to accept referrals from a variety of resources via phone and fax.

    Required qualifications:

    • Coursework/Training: Completes necessary education to attain LVN Licensure in state of California.
    • California Licensed Vocational Nurse.
    • 1 year Acute care experience.
    • 1 year Home health experience.
    • Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information.
