    Massachusetts Case Manager RN - Contract (Remote - MA ONLY)

    This position is a fully remote contract role available exclusively to candidates who reside in Massachusetts.

    Seeking a dedicated and skilled Massachusetts Case Manager RN for a fully remote, contract role offering $45/hr. This vital position supports members with complex healthcare needs by implementing effective care management strategies. The role allows for independent work while also participating in a multidisciplinary team effort, requiring candidates to reside in Massachusetts and hold active RN licensure.

    Responsibilities:
    • Engage and manage care plans for members, focusing on compliance and goal-setting
    • Apply case management standards to document comprehensive management plans
    • Educate members on medication and manage complex pharmacy needs
    • Maintain engagement with members to support health outcomes and care transitions
    • Liaise with healthcare providers to support comprehensive patient care
    Qualifications:
    • Active RN licensure in Massachusetts required
    • Must be residing within the state of Massachusetts
    • 3-5 years of experience in diverse clinical settings
    • Strong understanding of utilization management preferred
    • Proficient with multiple IT systems and adaptable to change
    • Excellent communication skills and ability to work with a diverse population
