    JOB SUMMARY: While assisting the Intake District Manager with the carrying out the vision of the agency, this position is responsible for receiving/processing intakes referrals for all four counties within the San Andreas Regional Center catchment area. This is an essential position, which supports the entire Intake Unit in providing qualitative and professional services to applicants, consumers, their families, and the community.

    ESSENTIAL JOB FUNCTIONS:

    • Officer of the Day Duties
    • Has expertise in regional center eligibility criteria, services, and due process.
    • Receives, processes and follows up on all intake referrals received via phone, walk-ins, website, electronic mail, postal mail, and fax within specified time, etc.
    • Provides information and education to those individuals being referred and those referring.
    • Creates a SANDIS profile, inputs demographics, and assigns inquiry numbers for new applicants for all counties.
    • Maintains up to date information on all new referrals whether inquiry or UCI.
    • Collects records needed to submit a complete file for case assignment.
    • Properly assesses and determines the next steps needed by individuals to provide a successful referral.
    • Consults with Intake District Manager and clinical team for case reviews as needed.
    • Assures that all applicants and families are informed verbally and in writing of their right to appeal every time a decision is made to change, alter, or deny a service, including eligibility for an intake assessment.
    • Is an expert on generic resources and assists with developing this knowledge among the intake service coordinators.
    • Knows Title 17 and Title 22 licensing and DDS requirements.
    • Has knowledge of the SANDIS case management system and use of Excel.
    • Travels to all four counties in catchment area as needed.
    • Performs other related duties as assigned to support the Intake staff, District Manager, and clinical team.

    EDUCATION/EXPERIENCE: A Bachelors Degree in social work, rehabilitation counseling or a related field from an accredited college. A minimum of five years of service coordination experience, or a minimum of 2 years of Intake Unit service coordination experience.

    SPECIAL SKILLS AND KNOWLEDGE: Knowledge of the agency; knowledge of the laws, rules and regulations governing the operation of the agency; knowledge of the principles, objectives and techniques of social work, individual and group behavior, human growth, development of family relationships; medical-psychosocial, socioeconomic factors affecting individuals and families. Knowledge of computer skills, strong/effective written and verbal communication with the ability to handle adverse conversations that may arise.

    OTHER: Use of an automobile, possession of a valid drivers license and automobile liability insurance for the minimum amount prescribed by law or the ability to provide independent transportation.
