Job summary
This role focuses on assessing the needs of patients who have physical, mental or emotional disabilities and delivering therapy services to improve their developmental, physical or cognitive impairment.Evaluates patients admitted with COPD diagnoses to determine the need for education and follow up.
Collaborates with care teams including physicians nurses respiratory therapists case managers social workers etc. To provide plan of care including but not limited to COPD action plans education smoking cessation medications oxygen therapy delivery devices advanced care planning pulmonary rehabilitation diagnostic testing.
