- Provide appropriate intervention services to patients and families
- Assess patient and family needs and concerns related to illness and treatment options
- Offer emotional support and develop service plans, including advocacy and referrals
- Consult, orient, and train healthcare staff on cultural and economic factors affecting patient care
- Master's degree in Social Work from an accredited college or university
- Two years of relevant work experience
- Experience in medical social work and social services delivery systems
- Knowledge of assessment, interviewing, and counseling techniques
- Collaborate with healthcare teams to address patient needs
- Conduct assessments to evaluate social, emotional, and physical needs
- Develop patient management plans and make necessary referrals
- Help patients and families cope with illness-related challenges
