Assesses and monitors the client's on-going psychiatric, medical and social needs.
- Plan, implement and evaluate programs of care and therapeutic intervention to meet the needs of an identified caseload of chronically mentally ill individuals.
- Act as case manager both to clients on own caseload and that of other professionals on the team.
