Integrity, Teamplay, 1% Better Each Day SUMMARY The Case Manager provides group therapy and related documentation services, along with additional ancillary services.

The Case Manager is a highly qualified individual with experience regarding the nature of alcoholism and drug addiction.

The Case Manager will need to be familiar with 12-Step Recovery programs and have knowledge of documenting group notes in an EMR system.

Must have the ability to work as a team member and work collaboratively with the Primary Counselor and Clinical Director in providing quality treatment services and client care.

ESSENTIAL DUTIES AND RESPONSIBILITIES Coordinate and provide care that is safe, timely, effective, efficient, equitable, and patient-centeredHandle case assignments, draft service plans, review case progress and determine case closureCommunicate patient needs to a variety of care team members and follow up accordingly.

Manage discharge plans upon completion of treatment.
Attends clinical and administrative meetings for inter-departmental sharing of information.

Work collaboratively with patients, families, therapist, and referral sources to ensure high quality careFacilitate multiple care aspects (case coordination, information sharing, etc.)Maintain the patient's comprehensive clinical record through detailed documentationHelp patients make informed decisions by acting as their advocate regarding their clinical status and treatment optionsPromote quality and cost-effective interventions and outcomesRecord cases information, complete accurately all necessary forms and produce statistical reportsHelps navigate legal issues that patients are currently facing up to an including appearance in court hearings.

Adhere to professional standards as outlined by protocols, rules, and regulationsAct as the patient's advocate as it relates to insurance coverage and financial assistanceContinually monitor and evaluate each client's progression through their case management plan, and develop corrective action revisions to the plan as neededArrange abeyance with social services, health, and governmental agenciesPerforms other duties as assigned by the supervisor, which are consistent with the position and in compliance with agency policies and procedures.

Exposure to clients infected with Hepatitis B, HIV, TB, or other Infectious Diseases RequirementsEDUCATION / EXPERIENCE 1 to 2 years of experience as a group facilitator in Substance Abuse and Mental Health settingsBachelor's Degree in human services field, preferredMinimum 1 year of full-time employment working in human services or in behavioral health sciencesKnowledgeable of Department of Children and Families (DCF) guidelines and Joint Commission standardsMust be familiar with 12 Step Recovery programsKnowledge of Kipu, preferred Salary Description $35,000 - $45,000 yearly
