SUMMARY:
This position is primarily responsible for managing the care of patients, promoting effective education, self-management support, and timely health care delivery by performing the following duties.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Core duties and responsibilities include the following. Other duties may be assigned.
Serve as liaison between resident, family, physician and other health care providers regarding health care issues.
Maintains a current knowledge of OSHA and state regulations and routinely monitor community compliance with regulations related to resident care.
Provides follow-up contact with patient to ensure compliance with recommendations.Educates and involves patients to improve their health-patient engagement.
Assists patients to set goals, progress and identify barriers when patient not meeting treatment goals
Manages many aspects of patient's care: referrals, hospitalizations, ER visits, ancillary testing and other as directed to achieve optimum outcomes in safe and cost-effective manner.
Develops a list of medical supply and community resources available to patients.Operates department(s) within established budgetary guidelines.
Coordinates continuity of care with patient and or family to ensure compliance
Provides clinical follow-up with patients, giving information and guidance to enhance patient-care team communication
Maintains accurate and timely documentation
SUPERVISORY RESPONSIBILITIES:
This job has no supervisory responsibilities.

EDUCATION AND/OR EXPERIENCE:
Bachelor's degree (B.A.) from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.
