    Millennium Home Care (an affiliate of Millennium Healthcare, LLC)

    Are you a passionate and skilled Physical Therapist looking for an exciting opportunity in the field of home health? Millennium Home Care, a leading provider of compassionate and personalized healthcare services, is seeking a full-time Physical Therapist to join our dynamic team.     Responsibilities:

    Conduct comprehensive evaluations of patients' physical abilities and limitations

    Develop and implement individualized treatment plans to improve mobility, strength, balance, and overall functional independence

    Provide skilled therapeutic interventions, such as therapeutic exercises, manual therapy techniques, and assistive device training

    Educate patients and their families on proper body mechanics, home exercise programs, and injury prevention strategies

    Collaborate with interdisciplinary healthcare teams to ensure coordinated and holistic care

    Document and maintain accurate records of patients' progress and treatment plans

    Qualifications:

    Active and unrestricted Physical Therapist license in the state of Florida

    Minimum of 1 year of experience in physical therapy, preferably in a home health or a related setting

    Strong clinical skills and ability to provide patient-centered care

    Excellent communication and interpersonal skills

    Ability to work independently and make sound clinical judgments

    Proficiency in electronic medical record (EMR) systems

    Valid driver's license and reliable transportation to travel to patients' homes.
