- Conduct comprehensive medication reviews (CMRs) with patients to identify medication-related problems
- Provide targeted medication interventions (TMEs) and follow-up to support adherence and safety
- Collaborate with prescribers, health plans, and care teams to resolve therapy issues
- Document interventions clearly and maintain compliance with MTM program requirements
- Educate patients on safe effective use of their medications address barriers to adherence
