    The Homecare manager (LPN) is responsible for the development and implementation of homecare services and management of homecare staff, this includes during and after business hours.

    Directly oversee and manage homecare staff including but not limited to hiring, training, scheduling, and disciplinary action.
    Participate actively in Quality Improvement (QI) processes and complete annual competency checks for all Homecare CNAs and PCAs.
    Share every other week on-call responsibilities with the Homecare Manager for availability for after-hours, weekends, or holidays.
    Conduct monthly CENA in-home supervision visits to ensure care plans are being followed.


    Operations:
    Manage 24-hour care delivery in the home and ensure all home care services are implemented effectively.
    Oversee the coordination and implementation of GT Independence and Life Alert agency services.
    Cover CNA and PCA responsibilities as needed to ensure compliant home care services.
    Document services in the Electronic Medical Record (EMR)

    Care Plan & Coordination:
    Participate as a member of the Interdisciplinary Team, attending staff meetings and taking part in participant care planning.
    Assist in the development and revision of enrollee's plan of care as a member of the interdisciplinary team.
    Communicate participant changes with team members and ensure care plans are updated accordingly.
    Coordinate and schedule homecare services in collaboration with the Interdisciplinary Team (IDT).
    Must have one (1) year experience as a practicing nurse, preferably in either home or community health.
    Meet a standardized set of competencies for the specific position description established by Thome PACE and approved by CMS before working independently
