    Position Details

    The Adult Mental Health Case Manager is responsible for managing, assessing and assisting persons with severe and persistent mental illness and administering a variety of social services to county residents in need.

    Managers are proactive in identifying patients with complex needs within the community in collaboration with all relevant services and work with clients to enable independent living within the community and to avoid hospitalizations.

    Direct Service Duties and Responsibilities


    • Meets with clients to provide on-going support, counseling and assessments to assist in determining needs, strength and goals which includes: interviewing people with mental illness, referring clients to a variety of support services such as emergency services, mental health hospitalization, IRTS facilities, adult foster homes or board and lodge services.
    • Collaborates with other community agencies and resources and refers clients to various service providers. Identifies available and new resources and coordinates successful services for clients and their families.
    • Assists clients with completion of paperwork in order to maintain or apply for various benefits.
    • Monitors cases by verifying clients' attendance, observing and evaluating treatments and responses, advocating for needed services and entitlements, obtaining additional resources, intervening in crises and providing personal support.
    • Monitor clients' finances as representative payee, create budget with client, monitor expenses monthly.

    Administrative and Other Duties and Responsibilities


    • Develops case plans with obtainable goals, assess client progress in various programs. Documents client performance by maintaining current and accurate case notes and functional assessments. Log events and progress in case notes.
    • Prepares and maintains a variety of records related to case management including treatment plans and related documents.
    • Collaborates with other county departments to investigate problems and determine needs in the overall administration of social services and mental health services to community members including court petitions for civil commitment or guardianship.
    • Organizes and provides education, community outreach services and public relations regarding department programs and the role of an adult mental health case manager.
    • Collaborate with other agencies regarding mutual clients and provide expert advice and opinions to colleagues working within other clinical areas.
    • Carry CVCC/ AP on-call phone rotational 1 week schedule.
    • Communicates clients' progress by participating in weekly interdisciplinary meetings disseminating results and obstacles to therapeutic team.
    • Assesses needs of new clients by reviewing records and applications, determines need for evaluations and plans.
    • Maintain training/educational competencies and own clinical professional development.
    • Participate in audit projects to develop services to improve client care and quality of delivery.

    While these are the primary focus of the position, we believe strongly in teamwork and employees will be called upon to perform a variety of duties as part of their role with the County.


    Position Requirements

    Knowledge, Skills and Abilities


    • Ability to make decisions on a regular basis that may have complex and significant variables.
    • Ability to work with limited supervision.
    • Knowledge of social services programs within the County and the state.
    • Knowledge and understanding of laws governing social services.
    • Knowledge of mental health issues, resources and service provision.
    • Ability to facilitate counseling.
    • Ability to write reports and maintain records.
    • Ability to work under limited supervision with general autonomy in determining how objectives are met.
    • Ability to deal with aggressive, irrational or out-of-control behaviors.
    • Ability to motivate behavior and gain trust of clients.
    • Ability to assess capacity, gain valid informed consent and work within a legal framework with patients who lack capacity to consent to treatment.
    • Effective communication skills, both oral and written.
    • Ability to use various computer and software programs.
    • Ability to work under limited supervision with general autonomy in determining how objectives are achieved. The supervisor generally sets operating benchmarks, goals, and objectives.
    • Work complexity involves considerable analytical ability to select, evaluate, and interpret data from several sources.
    • Ability to maintain frequent outside and inside contacts to carry out organizational programs in matters that require cooperation, explanation, and persuasion.

    Education and Experience


    • Requires a four-year degree in social work, sociology, psychology or a related field.
    • One to three years of experience.
    • May have a combination of education and experience to successfully perform job duties.

    Licenses/Certifications
    This position will require the acquisition and maintenance of the following additional certificates/licenses:


    • Bachelor's Degree
    • MN Merit Eligibility
    • Driver's License or evidence of equivalent mobility
    • DHS Case Management Training
    • Fraud, Abuse and Waste Training

    Physical and Mental Requirements

    This job typically requires:
    sitting, standing, walking, feeling, manual dexterity, grasping, talking, hearing, typing, and seeing. There is exposure to computer keyboards and video screens. This position is generally light-duty and may require the exertion up to 50 pounds of force.

    Working Conditions

    Work is performed both in an office environment and in the field. There is travel required to other facilities, clients' homes, meetings, training, etc. There is exposure to hostile, aggressive or irrational persons. Potential for exposure to body fluids.

    Addendum for Mental Health Case Management


    • Manages client through entire civil commitment process, including interviews, coordinating PPS meeting, screening process, reports to court, attending court hearing, and coordination of transportation.
    • After client is committed through the courts, case manager oversees client in the community to ensure they are following requirements of provisional discharge or stay of commitment of treatment plan.
    • Revocation of provisional discharge is initiated and pursued on case by case basis and dismissal of commitment is also managed by case manager.
    • At court request, investigate and review Rule 20.01 Cases for purposes of civil commitments.
