    Island Nursing Rehab Center -

    We are a 120-bed not-for-profit nursing facility offering advanced healthcare while providing skilled nursing and rehabilitative services for both long and short term residents. Located on eight beautiful landscaped acres, we also provide sub-acute services, wound care and respite services.

    We are currently seeking a full-time Nursing Unit Secretary/C.N.A. who will embody the passion and commitment required to deliver the ultimate care for our residents.

    QUALIFICATIONS:

    Candidate must be a High School Graduate or GED equivalency. Must possess excellent typing and computer skills. Must possess two years' experience as a Unit Secretary in a similar setting is strongly preferred. Ability to read, write and verbally communicate with proficiency in English. Possession of current NYS C.N.A. certificate required.

    GENERAL OBJECTIVES:

    The Unit Secretary performs all general clerical and receptionist duties required for the functioning of the resident/patient care unit. The Unit Secretary performs all filing, alphabetizing, typing, answering phones, and all related clerical responsibilities. Possesses competency in thinning charts and knowledge of determining which paperwork from patient's medical record on floors can be relocated to the medical records department. Must possess excellent computer skills and the ability to type 30-40wpm.

    SPECIFIC RESPONSIBILITIES:


    1. Performs all receptionist and clerical duties
    2. Assist is Central Supply
    3. Ability to handle heavy phone volume and multi-task in an organized and efficient manner.
    4. Receive and direct residents/patients, staff, visitors etc. in a pleasant and professional manner.
    5. Ability to answer routine questions and/or refer to the shift Supervisor/designee for appropriate answers.
    6. Receive, sort and coordinate the delivery of all mail received by the unit in a timely manner.
    7. Transmit messages accurately and efficiently.
    8. Arrange routine resident/patient appointments as requested/document on calendar.
    9. Confirm all patient appointments. Check for any special preparation required.
    10. Perform all clerical duties related to maintenance of resident/patient charts and for efficient operation of patient care unit.
    11. Receives and attaches reports: laboratory slips, physician's orders etc. (verifying appropriate sign-off signatures(s) prior to filing).
    12. Thin(s) charts and maintains Medical Records charts as necessary according to established procedure. Filing, alphabetizing and typing all unit correspondence as required.
    13. Perform duties related to admission, discharge and transfer procedures.
    14. Prepare resident/patient identification bands per established procedure/replaces identification bands as needed.
    15. Prepare resident/patient chart, name, room number, heading/labeling all new sheets.
    16. Label eyeglasses, and/or other resident property as required and indicated.
    17. Prepares dietary slip(s) and notifies Food & Nutritional Services Department.
    18. Notifies appropriate departments of admission, discharge, transfer, expiration and condition changes per established procedures.
    19. Processes discharge chart for medical records.
    20. Accurately generates all referral forms for PT, OT, X-ray, EKG, etc. in an efficient manner.
    21. Responsible for calling physicians offices for laboratory/EKG requests.
    22. Prepares routine unit forms such as weight sheets, vital sign sheets, medication sheets, treatment sheets, I & O sheets and all related forms as indicated by unit and departmental needs.
    23. Responsible for the requisition office supplies and forms for the nurse's station to ensure efficient operation of the unit.
    24. Required to transport residents to and from service areas, e.g., beauty parlor, podiatrist, dentist as requested.
    25. Coordinates and maintains the physicians "rounds" book. Contacts each physician on a 28 day cycle informing them of schedule of residents due to be seen. Forwards to each physician a copy of the residents they are due to see.
    26. Arranges transportation for residents/patients to and from scheduled appointments outside of facility.
    27. Demonstrates knowledge, respect and sensitivity of resident's rights as identified within the Resident Bill of Rights and facility policy and mission statement. Assures confidentiality of all resident information.
    28. Place appropriate blank consent forms in the chart.
    29. Check chart for current lab slip and transmit information to licensed nurse.
    30. Check chart for completeness before transfer of patient.
    31. Notify food service regarding diet changes, meal holds, etc.
    32. Order equipment, supplies and x-rays for special consult exam at the direction of the licensed nurse.
    33. Notify licensed nurse of consultant appointments.
    34. Performs all other related duties as required.
      PHYSICAL REQUIREMENTS: As required by the Americans with Disabilities Act (ADA): a) to be able to walk, move, traverse, about b) to be able to ascend/descend, traverse stairs c) to be able to maintain a stationary positon ( sit/stand) for extended periods d) to be able to move, transport, remove approximately 5-10 lbs. (documents) e) must possess ability visually to detect, determine, recognize, inspect and auditory acuity to express, discern, convey, converse with or without assistive/corrective devices f) must exhibit the ability to communicate (transmit and receive), convey, exchange both verbal and written information in an effective time efficient manner g) must exhibit the physical ability to provide the full scope of required job duties. This includes but is not limited to lifting, bending, twisting, prolonged sitting and repetitive movements h) must demonstrate the ability to comprehend verbal , written and auditory instructions and follow through with logical plan and course of action i) must demonstrate the ability to perform assigned tasks independently without requiring constant re-direction j) ability to perform heavy lifting ( 50lbs.+), push wheelchairs and stretchers k) ability to position self to ( bend, twist, turn, push, pull) carts, beds, wheelchairs, geri-chairs, etc.) l) ability to assist in the mobility of residents/patients and equipment m) ability to work in a direct resident/patient care setting HOURS OF WORK: HOURS TO BE DETERMINED
