Job summary
As a member of the Patient Centered Medical Home Team Pediatric Nurse Practitioner assists in creating partnerships between individual patients seeking care at the Santa Rosa Community Health and the providers by delivering excellent comprehensive primary health care prevention and health education.
Duties include taking medical history performing physical exams making assessments diagnoses initiating treatment therapy plans including developmental assessments preventative well teen and child care family planning care promoting childhood vaccines maintaining documentation ordering interpreting laboratory reports diagnostic imaging coordinating health management with physicians other agencies meeting regularly with Medical Director or Attending Physician to review charting medical provided.
- Create an ongoing relationship with each patient
- Maintain adequate documentation of all patients seen including follow-up referral in electronic medical record
