Nurse Practitioner Monogram Health is looking for skilled Nurse Practitioners and Physician Assistants eager for the opportunity to make a difference in patients' lives.The Advanced Practitioner at Monogram Health is a key member of an integrated Care Team which includes a Registered Nurse and a Social Worker.

Responsibilities
Conducts assessments comprehensive annual wellness exams on patients both in the patients home and in the virtual environment Counsels educates patients families about benefits programs available to help them live healthier lives Documents items such as chief complaint all applicable diagnosis past medical family social history review of systems examinations medications allergies assessment plan Responsible for coordination of care primary care providers specialists ancillary services Completes all documentation paperwork timely Maintains quality standards defined by practice Position will not be office-based but will remote state employed need attend periodic trainingmeetings outside that state Deliver evidence-based timely care reduces avoidable hospitalizations maximizes quality life puts patient health satisfaction first Prescribe medications order tests collaborate patientMonogram physician Perform effectively reflected improved patient quality outcomes measured reported daily Facilitates closing gaps care educating patients preventive monitoring working physician practices schedule diagnostic testing Assists enrolling access educational videos Participates integrated team meetings Knowledge disease diagnosis prevention Make assessment health status Develop treatment Implement plan consistent appropriate plan follow-up evaluate status Other duties assignedJob description
