This role includes conducting home safety and aging-in-place assessments strength and mobility check-ups daily self-care and home-management reviews screenings for functional fitness and endurance as well as supporting clients through lifestyle habit meaningful activity reviews emotional well-being quality-of-life check-ins. This position offers personalized lifestyle wellness consultations to help clients maintain independence thrive at home.
