- Master's Degree Social Work is required.(Individuals hired as a CTSW prior to May 11, 2017 will be grandfathered to the CTSW position with BSW, at the entity they were employed at on May 11, 2017)
- Three (3) years in hospital/medical social work is highly preferred.
- One (1) year of discharge planning/care management is preferred.
- LMSW - Licensed Master Social Worker orLCSW - Licensed Clinical Social Worker is required upon hire.
- CPR - Cardiopulmonary Resuscitation is required upon hire.
- ACM - Accredited Case Manager is preferred upon hire or
- CCM - Certified Case Manager is preferred upon hire.
- ANCC certifications also preferred.
- Facilitates care conferences for complex transitions, placement and palliative care needs.
- Proactively identifies patients who no longer meet continued stay criteria and communicates with the physician team.
