Heart Care Transition Clinic - Fort Myers

Job summary
The Heart Care Transition Clinic is a specialized medical office for cardiac patients recently discharged from the hospital to receive follow-up medical care.The position is primarily based out of our Metro Parkway and HealthPark Medical Center, however training and orientation will require floating to all 4 of our acute care facilities. The schedule is Monday - Friday with 8-hour shifts (the practice may consider a 4-day work week with 10-hour shifts for the right candidate)
This role would be responsible for working with cardiologists to ensure patients get the follow-up care needed in a timely manner. These clinics focus on patients with a diagnosis of Heart Failure, AFib, and other chronic cardiac conditions to provide follow-up care, medication and diagnostics review and focus on in-depth patient education as a means to assist patients with diet and lifestyle changes as a means to liaise care back to the patient primary cardiologist and mitigate readmissions.
