We currently have a vacancy for a Wellness Navigator to work a Full schedule. In this position a Wellness Navigator will be responsible for building relationships, solving problems and locating resources for residents transitioning throughout the life plan community. The Navigator will evaluate residents' needs and assist them in accessing the available resources needed to ensure a seamless transition between appropriate levels of care offered at the community, in addition to but not limited to the following duties:

Social Work or Clinical Assessment background is a plus Prefer supervisory experience in health care or a long-term care facility with skilled and support staff. Ability to work extended or flexible hours, as necessary. Ability to read, write, speak and comprehend written and oral instructions in English.
