    TheValue-Based Care Coordinator (VBCC) willfacilitate, implement, and support strategies to optimize organizationperformance in Value-Based arrangements.

    Under the direction of the Manager,Value-Based Care, the Value-Based Care Coordinator will provide collaborativeownership and execution on defined Value-Based/Quality Programs includingquality metrics, care coordination, clinical documentation, and support forHoneycomb patients.

    The primary function of this role is collaboration with theQuality/Value-Based Care team to implement strategies and workflows that driveimprovement in performance and quality.

    This position is ideal for aresults-oriented self-starter with strong interpersonal skills. It is vitalthat the right candidate has flexibility to adapt to changing priorities andwork.


    Dutiesand Responsibilities:


    TheVBCC is responsible to assisting the organization with improving the overallperformance on quality benchmarks in Value-Based arrangements throughactivities such as identification, outreach, coordinating patient visits,conducting chart reviews, quality measure education, and assisting with datasubmission to payers.


    • Demonstrateunderstanding of Value-Based care principles especially as they relate topopulation health management and improvement in health care quality.
    • Demonstratea strong knowledge of Value-Based/Risk Adjustment (HCC) documentation andcoding guidelines.
    • Reviewperformance data (i.e. quality, risk adjustment, care coordination) forassigned centers and collaborate with Quality team on action plans that supportorganizational goals. Build and develop collaborative relationships with centerteams to drive performance.
    • Engagein patient EMR chart reviews to help improve clinical documentation and closequality gaps.

    • Musthave a valid driver's license as individual will be required to travel betweencenters for project work.
    • Acceptsother duties, as required, willingly. Demonstrates follow through on assignedduties.

    Core Competencies:

    • Completes tasks and projects needed toensure all contracted Value-Based compliance with applicable rules andregulations defined by governmental agencies and commercial payers.
    • Works closely with the Manager,Value-Based Care to collaborate and potentially facilitate project scope,monitor timelines and deliverables, and communicate and identify pathways toresolve risks and barriers.
    • Utilizes solid business planningprinciples to develop systems and processes that produce sustainable resultsand support ongoing operations of Value-Based care.
    • Provides timely post facility dischargefollow up that includes but not limited to Medication Reconciliation, PostDischarge instruction review, Confirmation of completion of any communityreferrals, Primary Care Physician or Specialist Follow up Visit scheduled perTransit Care Management Policy.
    • Responsible for quality measureimprovement through education and review of quality alerts within theElectronic Medical Records, Chart Review, and payer attestation.
    • Facilitates patient access to appropriatemedical and specialty providers by scheduling Valued-Based Annual WellnessVisits (AWV) and other appropriate chronic care, medication compliance, andhealth maintenance needs.
    • Consistently presents a professionalappearance in dress and grooming by adherence to dress code policy.
    • Adheres to Health Insurance Portabilityand Accountability Act (HIPAA Privacy Rule) policies and procedures.
    • Adheres to policies/safety practicesestablished by Honeycomb Management Services and its designated agents andrepresentatives.
    • Upholds, complies with, and enforces theCore Principles and Code of Conduct

    MinimumQualifications:

    • A graduate of an accredited MedicalAssistant (MA) or nursing school and holding certifications and/or licensure isrequired.
    • Two years clinical experience inValue-Based care is preferred.
    • Proficiency in Microsoft Excel ispreferred.
    • Must be proficient in Electronic MedicalRecords, Electronic Health Records, and Health Information Technology systems.
    • Two years risk adjustmentcoding/documentation experience is preferred

    License/Certification:
    Certified Medical Assistant (Required)

    BLS Certification (Preferred) CPR Certification (Required)PhysicalRequirements:
    Thephysical activities of this position may include climbing, pushing, standing,hearing, walking, reaching, grasping, kneeling, stooping, and repetitivemotion. Must have good balance and coordination. The physical requirements ofthis position are medium work - exerting up to 50 lbs. of force occasionallyand/or up to 25 lbs. of force frequently.

    The Value-Based Care Coordinator isrequired to have close visual acuity including color, depth perception, andfield of vision to perform an activity, such as assessing patients, andanalyzing data and figures; transcribing; viewing a computer terminal; orextensive reading.
