- Provides timely post facility dischargefollow up that includes but not limited to Medication Reconciliation, PostDischarge instruction review, Confirmation of completion of any communityreferrals, Primary Care Physician or Specialist Follow up Visit scheduled perTransit Care Management Policy.
- Responsible for quality measureimprovement through education and review of quality alerts within theElectronic Medical Records, Chart Review, and payer attestation.
- Facilitates patient access to appropriatemedical and specialty providers by scheduling Valued-Based Annual WellnessVisits (AWV) and other appropriate chronic care, medication compliance, andhealth maintenance needs.
- Consistently presents a professionalappearance in dress and grooming by adherence to dress code policy.
- Adheres to Health Insurance Portabilityand Accountability Act (HIPAA Privacy Rule) policies and procedures.
- Adheres to policies/safety practicesestablished by Honeycomb Management Services and its designated agents andrepresentatives.

- A graduate of an accredited MedicalAssistant (MA) or nursing school and holding certifications and/or licensure isrequired.
- Two years clinical experience inValue-Based care is preferred.
- Proficiency in Microsoft Excel ispreferred.
- Must be proficient in Electronic MedicalRecords, Electronic Health Records, and Health Information Technology systems.

TheValue-Based Care Coordinator (VBCC) willfacilitate, implement, and support strategies to optimize organizationperformance in Value-Based arrangements.Under the direction of the Manager,Value-Based Care, the Value-Based Care Coordinator will provide collaborativeownership and execution on defined Value-Based/Quality Programs includingquality metrics, care coordination, clinical documentation, and support forHoneycomb patients.
