Case Manager (CASAC-T, CASAC, LMSW, LMHC, LCSW), Outpatient *SIGN ON BONUS ELIGIBLE for external candidates*Arms Acres is looking for a Clinical Case Manager to join our outpatient team in Carmel, NY.

DUTIES AND RESPONSIBILITIES:
Provides/ensures comprehensive evaluation of patient's functioning as defined by facility policy and proceduresCoordinates the development and implementation of the multidisciplinary treatment plan; coordinates treatment plan review; performs these activities within established time framesDocuments patient progress and own interventions in case record and works within time frames of regulatory requirementsProvides individual, group and family treatment (psycho-education, life skills) as identified in patient treatment plans and requested by program directorPerforms utilization review, provides progress reports and develop aftercare plans in conjunction with referring persons/agencies, EAPs and managed care/insurance entitiesParticipates in facility program and quality improvement activitiesOther duties as assigned Requirements
REQUIRED QUALIFICATIONS:
CASAC, MSW, LMSW, LCSW or LMHCMaster's Degree in related fieldExperience working specifically with outpatient substance abuse counseling preferredBi-lingual preferredEOE AA M/F/Vet/DisabilityINDHPSalary Description $26.00-$35.
