- Master's degree or higher.
- 3+ years of experience in any of the following:
- Primary Care
- Value Based Care
- Valid Nurse Practitioner (NP) license in Virginia (VA).

Position Summary:
We are seeking a licensed Nurse Practitioner to lead the medical aspects of team-based care for individuals with serious mental illness. This role involves documenting and supporting individuals' medical needs through comprehensive clinical assessments, supporting care coordination, and providing clinical guidance to team members.
Responsibilities:
- Conduct comprehensive clinical assessments, gather information, assess both behavioral and physical health conditions, make recommendations or referrals, and provide support as needed.
- Lead the medical component of team-based care, including training team members, supporting care coordination, identifying care gaps, performing medication reconciliation, and communicating with care providers.
Qualifications:
To excel in this role, you should possess empathy, compassion, excellent communication skills, and a desire to help others. You should be comfortable working in a multidisciplinary team, have cultural humility, and be familiar with local healthcare providers.
Experience:
Ideal candidates will have at least 5 years of clinical care experience as a Nurse Practitioner, experience working with underserved populations, and proficiency in care management and coordination.
