    Compensation:
    Salary $80,000 - $90,000 for full-time employees, with extra earning potential

    Paid OrientationOn-Site Training and ShadowingMileage ReimbursementGenerous PTO policyCDPHP health benefits401kLife insuranceTablet and Cell phone providedWhat you will do:Manage, supervise and evaluate clinical staff, conduct supervisory joint field visits with clinicians, perform clinical record reviews, audit for compliance standards and case conference with interdisciplinary clinical team.

    Oversee the assignment of patient visits to Primary Care Nurses and Home Care Nurses and the overall distribution of caseloads.

    Assist primary care nurses in coordinating care with other colleagues among the interdisciplinary team.
    Serves as Case Manager as needed.
    Interpret agency policies and procedures to staff, patients and community agencies.
    Triage phone calls from patients and assess clinical problems directly with them and/or indirectly with their caregivers.

    Be available during working hours of the agency to handle physician calls, accept and transcribe new MD orders, communicate these orders on behalf of the Primary Care Nurses and assist with intake of new patients.

    Case conference with Service Coordinators and other caregivers for patients when Primary Care Nurse is unavailable.
    Supervise scheduling of patients and staff in concert with Manager of Intake and Scheduling.
    Advise Director of Patient Service and Executive Director regarding staffing needs.
    Investigate patient complaints and patient/employee incidents and report findings to the Director of Patient Services.
    Coordinate staff development through orientation and in-service programs and individual guidance.

    Assist the Director of Patient Services in ensuring quality patient care and maintenance of Department of Health/CMS regulatory requirements and agency standards.

    Recommend advancement, retention or termination of employees under his/her supervision.
    Participates in Quality Improvement initiatives and activities.
    Required participation in on-call rotation as determined by agency policy.
    Maintain clinical knowledge and skills; enhance own professional growth and development by attendance at conferences and programs.
    Make patient visits when cases are uncovered or extra visits are necessary.
