    Saint Elizabeth Home location, a 168 bed skilled nursing and rehab center is currently hiring an RN MDS Coordinator for Full Time Day.

    The RN MDS Coordinator schedules, coordinates, directs, oversees, and participates in the completion of the Resident Assessment Instrument (MDS, RAPS, CARE PLANNING) in accordance with federal and state regulations as well as standards for long term care facilities established by the Joint Commission on the Accreditation of Healthcare Organizations.

    Ensures MDS and RAPS are completed accurately, and care plans evolve from comprehensive, multidisciplinary assessments.


    Qualifications for an RN:

    MDS Coordinator:
    Must be a Registered Nurse licensed by the state of Rhode Island.

    Must have, as a minimum, one (1) year experience as a supervisor in a hospital, long-term care facility, or other related health care facility.

    Must have 2-3 years geriatric nursing experience.
