    Estimated Salary: $44.00-$44.00 per hour

    Summary:

    The primary emphasis of this role is to partner with the healthcare team to ensure efficient patient care, minimizing delays and denials whenever possible.

    This position requires a lead role in coordinating with community agencies to address various social issues, such as abuse, neglect, substance abuse, mental health, or adoption, and linking patients to financial resources.

    Responsibilities:

    1. Partner with the healthcare team to ensure efficient patient care.
    2. Coordinate with community agencies to address social issues.
    3. Link patients to financial resources.
    4. Maintain accurate records and reports.

    Requirements:

    • Master's degree in Social Work required.
    • 2-4 years of Social Work experience required.
    • Recent Social Work experience in Medical Social Work is preferred.
    • LCSW-C - Certified Clinical Social Worker required.
