    Job Summary:

    Position Summary:

    Provides high quality professional nursing services in a home care setting. Reports to the Home Health Administrator.

    Position Qualifications:

    Minimum Education: Post high school vocational/specialized training

    Minimum Experience: 1 year of med/surg or acute care experience in an

    institutional setting

    Preferred Experience: 1 year of prior home care experience

    Required Course(s)/Training: Program for licensed practical nursing

    Required Certification/Registration: Current WV L.P.N. licensure (or eligibility) and

    CPR certification

    Skills: Other Good oral & written communication skills

    Physical Demands Prolonged, extensive, or considerable standing/walking

    Lifts, positions, pushes and/or transfers patients

    Lifts supplies/equipment

    Considerable reaching, stooping, bending, kneeling,

    crouching

    Working Conditions: Contact with patients under wide variety of

    circumstances

    May be exposed to/occasionally exposed to patient

    elements

    Exposed to unpleasant elements (accidents, injuries, and

    illness)

    Subject to varying and unpredictable situations

    Handles emergency or crisis situations

    Travel required
