    Job Summary:

    The hospice director of clinical services is responsible for managing the provision of patient care and services for each patient with a terminal illness regardless of age, race, religious affiliation or sexual orientation. Services provided are in accordance with recognized standards of practice and includes functions that pertain to palliative, supportive services required by a patient with a terminal illness, the philosophy, mission, policies, and procedures of the hospice service and as ordered by the physician. The director of clinical services assigns interdisciplinary group (IDG) members to teams and supervises the home health aide. The director of clinical services is also responsible for providing skilled nursing service.

    Qualifications

    Qualifications:
    • A current and unencumbered license as a registered nurse (RN) in Texas or in accordance with the Texas Board of Nursing rules for Nurse Licensure Compact (NLC).
    • Bachelors degree from an accredited school of nursing preferred.
    • At least one year of management or supervisory experience in a health-related setting, such as:
      1. A home and community support services agency.
      2. A hospital
      3. A nursing facility
      4. An outpatient rehabilitation facility
      5. A psychiatric facility
      6. An intermediate care facility for persons with mental retardation or related conditions; or
      7. A licensed health care delivery setting providing services for individuals with functional disabilities.
    • Thorough understanding of and support for the hospice philosophy.
    • Excellent interpersonal skills to relate effectively to staff, patients, families, and community.
    • Good interpersonal and written and verbal communication skills which will provide positive atmosphere for patient, family and primary casegiver(s).
    • Must be able to read, write and comprehend English.
    • Must meet the requirements on the Training in Administration of Agencies referenced in Texas Administrative Code (TAC) § relating to Initial Training in Administration of Agencies).
    • Valid driver's license, automobile insurance and own transportation to make home visits.
    • Must be capable of maintaining regular attendance and be available for travel to various locations of patients homes.
    • Must be capable of performing all of the essential job functions of this position, with or without reasonable accommodations.
    Essential Functions:

    The essential functions described below are representative of those an employee encounters while performing the basic functions of the position. The hospice Director of Clinical Services is at a minimum responsible for the following functions, either directly or through delegation to qualified staff:
    • Serves as Executive Director in his/her absence.
    • Participates in activities relevant to the clinical services provided, eg., policy and procedure developoment, revision and implementation, development of qualifications and assignment of agency personnel; hiring and orientation of new staff.
    • Develops and implements procedures for the timely evaluation of patients who are potential candidates for admission to hospice care, timely assessment of referred patients needs, and timely care planning for admitted patients.
    • Supervises, guides and directs clinical staff in the areas of poilicy and operation, caseload management and personnel management,
    • Performs and documents the initial and ongoing patient assessment to identify the patients and familys physical, psychosocial, safety, and environmental needs.
    • Develops, implements and revises the plan of care in collaboration with the IDG and the patients physician.
    • Ensures that a patients plan of care is executed as written.
    • Involves patient and family in the treatment plan.
    • Provides skilled nursing care to patients requiring professional nursing service; gives prescribed medications and other treatments.
    • Interprets the care plan and demonstrates appropriate procedures as necessary to licensed vocational nurses, nursing aides, other personnel, patient, family and primary caregiver(s).
    • Provides family-facilityed care across treatment settings, including performance of medical treatment prescribed by a physician for pain and symptom control and the provision of emotional support to the patient and family in accordance with the patients and familys spiritual and cultural beliefs.
    • Provides education and consultation with the patient, family and/or primary casrgiver(s) regarding the disease process and self-care techniques.
    • Monitors the patients progress, and determines when other disciplines need to be involved in the patients care, or the patient needs to be placed on a different level of care.
    • Initiates appropriate referrals to other disciplines and services.
    • Coordinates services with other disciplines and inpatient care facilities, as indicated. The coordination may be achieved by phone calls and on-site conferences, and should be documented in the patients record. The essential components of service coordination include the following:
      • Assessing need and planning for the required services, supplies, equipment, and pharmaceuticals ordered or approved by the attending physician or IDG
      • Assuring appropriate and timely completion of physician orders by all assigned staff
      • Documenting changes in the plan of care
      • Maintaining efficient communications with all care providers to ensure prompt transmission of significant information, which may require action or decision making
      • Coordinating with all involved, including contracted services, to unify and maximize their contributions
      • Communicating in planned and spontaneous conferences to evaluate progress and consider revision of goals to meet changing needs.
      • Documenting visits and conferences, provision of therapeutic care, achievement of progress, and other pertinent data in a complete and timely manner.
    • Informs the physician and other members of the IDG when the following occur:
      • The patients condition changes
      • The patients response to treatments or medications is not as expected
      • Changes in caregiver support or in the patients environment.
    • Assigns an appropriate health care pofessional to perform a reassessment of a patients needs:
      • When there is a significant health status change in the patients condition
      • At the physicians request
      • After hospital discharge
    • Evaluates the effectiveness and outcomes of nursing care and hospice services on an ongoing basis. Evaluation of patient/caregiver progress takes into consideration:
      • Skills and health care knowledge needed to assume independence in care
      • The patients functional status
      • Objective observation of the patients response to treatment, including management of pain and other symptoms.
    • Establishes and maintains efficient methods of ensuring the medical necessity and appropriateness of all hospice admissions and discharges.
    • Performs concurrent reviews for patients to ensure that extended stays are medically justified and are so documented in patient's medical records.
    • Supervises licensed practical/vocational nurses (LPN/LVNs) and paraprofessionals providing patient services.
    • Develops and implements the continuing education program for hospice personnel.
    • Participates in and provides ongoing in-service education programs.
    • Aids families and primary caregiver(s) to accept and adjust positively from patient care to bereavement services.
    • Identifies problems and corrective measures related to the quality of patient care and overall delivery of hospice services and refers such problems and action plans to the Quality Assurance Committee.
    • Has reviewed Elysian Hospice Patient and Family/Casegiver(s) Bill of Rights and Responsibilities and knows the employees responsibility for enforcement of same.
    • Follows precautionary infection control practices (hand washing, mask, gloves, aprons) when providing care.
    • Participates in hospice IDG meetings.
    • Is available to the agency at all times in person or by telephone.
    • Participates in on call rotation as assigned.
    • Keeps current on State and Federal regulations and other accrediting standards governing the hospice operation.
    • Provides positive customer relations in dealing with patients, families, physicians, fellow department employees, other hospice, residential hospice, hospital departments, referral resources, service agencies, and others.
    • Adheres to all hospice policies.
    • Follows precautionary infection control practices (hand washing, mask, gloves, aprons) as necessary, when providing services.
    • Responsible for assuring patient/resident safety.
    • Performs related duties as assigned.
