    $240,000 - $263,000 in [New York, NY]
    Varied salary range based on experience and location


    Role Description:


    The main goal of a Primary Care Physician at Oak Street Health is to offer high-quality, value-based healthcare to local medicare patient communities through our innovative network of primary care centers in neighborhoods.

    Our Primary Care Physicians are based at one medical center, supported by extensive care teams so providers can focus on delivering quality care rather than a high volume of services.

    The structured model of value-based care and competitive incentives aim to yield positive outcomes, minimize hospital admissions, provide preventive care, and offer an exceptional patient journey.

    We are seeking physicians who are driven to deliver outstanding care to seniors. Recent graduates are welcome to apply.

    Exceptional candidates may be considered for a role as Center Medical Director.

    Schedule:
    Full-Time, Monday to Friday, 8:00 AM - 5:00 PM


    Responsibilities:
    - Assess and diagnose patients at local clinics
    - Oversee primary care delivery
    - Prescribe medication and treatments
    - Manage Patient Electronic Medical Records through designated systems
    - Collaborate with leadership to achieve health quality objectives
    - Supervise and lead the care team at the center
    - Perform other assigned duties


    Required Qualifications:
    - Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) degree
    - Board Certification in Internal Medicine or Family Medicine (or eligibility)
    - Active, unrestricted State License without probation
    - Active DEA license
    - Authorization to work in the USA
    - Exemplifies the values of being "Oaky"


    Preferred Qualifications:
    - Completion of Geriatrics Fellowship
    - Experience in treating Geriatric patient groups
    - Familiarity with Value-Based Healthcare Models
    - Track record of collaborating in a healthcare environment for positive outcomes
    - Work experience in outpatient primary care settings
    - Bilingual abilities if applicable
