    Director of Social Work - Nursing Home

    We are seeking an experienced Director of Social Work for our nursing home in Stamford, CT.

    Responsibilities:

    • Plan and direct a comprehensive social services program to support residents and families.
    • Coordinate admissions and discharges to maximize facility capacity.
    • Participate in an interdisciplinary team approach to serve the needs of our population.
    • Develop and implement effective case management strategies to improve resident outcomes.
    • Supervise and mentor social work staff to ensure high-quality care.

    Requirements:

    • Bachelor's or Master's degree in Social Work (BSW or MSW).
    • Current state-issued license as a Licensed Clinical Social Worker in Connecticut.
    • Several years of direct social work experience in skilled nursing facilities, preferably with supervisory roles.
    • Strong organizational and case management skills.
    • Experience in long-term care.
    • Excellent communication and interpersonal skills.
