    This contract pays approximately $10,695/month gross.

    Assignment details:

    • Contract length: 13 weeks
    • Shift length: 8 hour shifts
    • Hours per week: 40
    • Start Date: ASAP

    Job responsibilities:

    • Work 40 hours per week with 8-hour shifts.
    • Facilitate care coordination across providers to support continuity of services.
    • Coordinate discharge planning activities and communicate plans with patients and care teams.
    • Screen for social determinants of health and incorporate findings into care plans as applicable.
    • Document assessments, plans, and communications in the medical record.
