    GENERAL SUMMARY


    Works within a multidisciplinary team to provide an array of direct (face-to-face) and indirect clinical services to adults in a person-centered, trauma informed, individualized focused approach utilizing evidenced-based practices delivered to fidelity.

    These services are delivered in multiple types of settings including field-based services in the individual's home or other identified locations as well as clinic-based services.

    Individuals served may have multi-system involvement such is Criminal Justice, Child and Adult Welfare, Veterans, Homelessness, etc.

    ESSENTIAL DUTIES & RESPONSIBILITIES

    • Coordinate coverage for planned absences in advance with supervisor.
    • Ensures clinical documentation is submitted in accordance with CHCS standards, and contract specific requirements.
    • Links and refers consumers presenting to clinic to appropriate services.
    • Participate in centralized scheduling process. (varies by program)
    • Participate in supervision and development opportunities including individual supervision, group supervision, and meetings/trainings as assigned.
    • Provide direct individualized clinical services 60% of hours worked.
    • Provides compliance and clinical updates to court. Makes recommendations for treatment, incentives and sanctions. (varies by program)
    • Provides direct clinical services & assessments; develops treatment plan for consumers. (varies by program)
    • Provides emergency case management assistance, as needed. (varies by program)
    • Provides outpatient treatment services, including individual & group counseling. (varies by program)
    • Performs other related duties as required.
    Specific Program Descriptions

    ABH Complex Care:
    Complex Care focuses on tailoring services to individuals that would benefit from a multidisciplinary, team-based approach.

    Our teams (AOT, ACT, FACT, and POWER) strive to build community connections and tackle stigma associated with severe mental illness.

    As a result, our teams work closely with consumers in a community-based approach, overcoming the barriers of multiple hospitalizations and treatment non-adherence.

    Complex Care team members are provided with the tools/skills necessary to advocate, assist, and motivate consumers to take part in their recovery.

    Complex Care Licensed Clinicians provide trauma informed therapeutic services utilizing evidenced based practices to empower individuals.

    Together, we work to integrate individuals back in the community, instill hope in recovery, and provide the additional support for mental health needs.


    Adult Crisis Services - The incumbent in this position works in the Adult Crisis Unit or the Josephine Recovery Center.

    Services in the unit include emergency and planned crisis care for adults ages 18 and over who have a behavioral health diagnosis or need.

    The Adult Crisis Unit program provides meals, medication monitoring, supervised recreation, skills training, community referrals, and 24-hour supervision in a secure environment.

    The Josephine Recovery Center is not a secure environment but provides the same services as The Adult Crisis Unit.

    In addition to these sites CHCS operates a Mobile Crisis Outreach Team (MCOT), a resource designed to bring a crisis worker into the community to provide a face-to-face assessment and intervention, follow-up and relapse prevention services.


    Adult Services - Outpatient - The adult outpatient program provides an array of psychosocial rehabilitative services, case management services, and counseling to individuals from vulnerable populations (veterans, people without homes, justice involved, etc.) with severe mental illnesses.

    Our clinicians maintain a caseload that is tailored to an individual's level of needs and measures ongoing progress through their recovery.

    Clinicians build critical skills to develop their career in mental health, while learning to serve individuals diagnosed with mental health and co-occurring disorders.

    We work with individuals through all stages of recovery; also focusing on community integration and building independence. Some team members provide services in the community, targeting greater access of care to our consumers.


    • Eastside Clinic Licensed Clinicians will assist with enrollments for individuals needing to be expedited to the Eastside Clinic the enrollments can be done on a hybrid basis (Using telemedicine to provide services remotely - This would include a modified schedule based on clinic need to be identified by Clinic Administration).
    • PEC Clinic Licensed Clinicians will assist with enrollments for discharged individuals who return to services within six (6) months from their last service date as well as transfers from children's services and MOMMIES Program.
    Assisted Outpatient Treatment (AOT)

    Unit 4422:
    Consumers are on a civil commitment for either 90 or 365 days as ordered by the Judge. Receive standard TRR services and are sometimes co-served with FACT/ACT teams when meeting criteria for a higher acuity LOC.

    After commitment expires consumers can renew commitment with AOT, or if not, can transfer to another ABH clinic or discharge to community services.

    AOT team meets with court team weekly for updated dispositions on select consumers. Primarily mixture of LOC-3 and some LOC-1's however can be LOC-4 and co-served with FACT/ACT.


    Adult Services- Enrollment Licensed Clinician - Provides the initial routine contact for Mental Health services via assessment and documentation for program eligibility, using current DSM criteria to render a diagnosis of mental illness/obtain diagnosis from collateral and/or from licensed clinician along with other clinical assessment tools to determine appropriate level of care and medical necessity.

    The Enrollment Licensed Clinician collaboratively develops Initial Recovery Plans with the consumer/legally authorized representative (LAR), assists with coordinating initial psychiatric evaluation appointments for outpatient services, links consumer / LAR to internal and external services / resources, and assists with crisis intervention/resolution when needed.

    The Enrollment Licensed Clinician position does not carry a caseload. This position involves cross training to work with both adult and children's populations to help support community needs.

    Mommies Program:

    The incumbent in this position works within an outpatient substance abuse treatment program targeting adult pregnant and existing mothers struggling with managing co-occurring psychiatric and substance use disorders.

    Services provided would initially include screening and assessment, along with diagnosing mental health and substance use disorders.

    Additionally, individual counseling, group counseling, couples/family counseling, crisis intervention, case management, and skills training are also associated with this role.

    Group courses provided by the program target relapse prevention, parenting, trauma, domestic violence, and life skills.

    The staff acts as an advocate for clients who are involved in medication assisted treatment, Child Protective Services, probation/parole, and specialty court programs.


    Adult Services- Justice Programs Licensed Clinician - The incumbent in this position will provide the initial routine contact for Mental Health services via assessment and documentation for program eligibility, using current DSM criteria to render a diagnosis of mental illness along with other clinical assessment tools to determine appropriate level of care and medical necessity.

    The Justice Program Licensed Clinician collaboratively develops Initial Recovery Plans with the consumer/legally authorized representative (LAR), assists with coordinating initial psychiatric evaluation appointments for outpatient services, assists with notifying case manager of new consumer added to their caseload to initiate initial engagement, and assists with crisis intervention/resolution when needed.

    The Justice Program Licensed Clinician also obtains authorizations to discuss information to allow for open communication with the various court teams and also any additional support system per the consumer choice This position also involves receiving services from various specialty courts and coordinating initial intakes with court team members, and in some cases will assist with completing screenings and coordinate the release of individuals from county or hospital facilities for attending their intake with CHCS.

    Justice Program Licensed Clinicians may serve the following programs:


    Community Reintegration Program (CRP): Consumers are on pretrial, having been released from the JIAA into services or having been directly referred by a Special Needs Unit (SNU) pretrial officer.

    Consumers receive standard TRR services over duration of pretrial status. Once pretrial is completed consumer is transferred to regular outpatient ABH within CHCS or can discharge to community services.

    Program duration is pending upon duration of pretrial status (can be anywhere from a few months to 2 years depending on the person charge, etc.).

    CRP team meets monthly with pretrial officers for updated dispositions on consumers. Mixture of LOC-1, LOC-2 and LOC-3.


    County Court 12 (CC12): Consumers are on probation, having been referred by the mental health court team under Judge Huff with County Court 12.

    Consumers are generally in the program for 1 year pending compliance with probation.

    Consumers receive standard TRR services and once probation/community supervision with CC12 court ends, they can transfer to another ABH clinic within CHCs or discharge to community services.

    CC12 team meets weekly with court team for updated dispositions on consumers. Mixture of LOC-1, LOC-2 and LOC-3.

    Justice Intake & Assessment Annex (JIAA)

    Programs:

    Jail diversion program designed for helping eligible arrested persons be diverted from jail and into outpatient mental health services within CHCS in efforts to reduce recidivism.

    Licensed Clinicians in this program screen individuals who are arrested and brought into the JIAA (Bexar County booking center) for possible mental health and/or substance abuse issues that have exhibited a mental health history as evidenced by various alerts via law enforcement screening and/or county computer systems.

    Clinicians assess individuals charged with Class A/B misdemeanors and felonies who has been identified with possible mental health and substance abuse issues, to divert them into outpatient treatment with CHCS instead of jail.

    Clinicians also assist with separating arrested persons with Developmental & Intellectual Disabilities (DID) from general population in the booking center and help them to be linked to AACOG services.

    Clinicians further complete assessments to comply with SB1326 (Sandra Bland Act), as well as crisis and risk assessments (Columbia Suicide Severity Rating Scale/CSSRS) for individuals who are/or may be meeting crisis criteria.


    Jail Based Competency Restoration Program:

    Offers legal education to individuals with mental illness who are residing in the Bexar County jail, and who have been found to be incompetent to stand trial.

    Staff utilize an evidenced based curriculum in a group and individual setting to help with restoring individuals to competency so that they can then stand trial and resolve their charges.

    Participants also work closely with psychiatrist, psychologist, and nurse staff to receive symptom stabilization through medication management, and ongoing assessment geared towards successful completion of the program.


    Community Alternatives to Incarceration Programs (CAIP):
    Felony Drug Court (FDC): Consumers are on probation for anywhere up to 2 years. Duration of program is pending completion of various phases.

    Consumers have a history of co-occurring mental health and substance abuse disorders and are primarily referred to CHCS for medication management.

    Also do receive case management and skills training services.

    Consumers are on the non-TRR 1115 Waiver ANSA however they do still receive case management with options for therapy services.

    Once probation is completed consumer can transfer to another ABH clinic or discharge to community services. CHCS FDC team meets with court team weekly for updated dispositions on consumers.

    Reflejo:
    Consumers are on probation for various lengths of time pending completion of various program phases.

    Consumers are on the non-TRR 1115 Waiver ANSA with 90 day Recovery Services Plan and receive medication management, skills training and case management services.

    CHCS team meets with court team biweekly for updates on consumer disposition. Upon successfully completion of program phases consumer can then transfer to another ABH clinic or discharge to community services.

    Treatment Recovery & Accountability Court - D (TRAC-D): Consumers are on probation for duration of various phases. Consumers receive medication management, skills training and case management services. Consumers are also on the non-TRR 1115 Waiver ANSA with 90-day Recovery Services Plan. CHCS treatment team meets with court team bi-weekly for update on consumer dispositions. Upon successful completion of the program phases consumer can transfer to another ABH clinic or discharge to community services.

    Treatment Recovery & Accountability Court - A (TRAC-A): Consumers are on probation for duration of various phases. Consumers receive medication management, skills training and case management services. Consumers are also on the non-TRR 1115 Waiver ANSA with 90 day Recovery Services Plan. CHCS treatment team meets with court team bi-weekly for update on consumer dispositions. Upon successful completion of the program phases consumer can transfer to another ABH clinic or discharge to community services.

    Esperanza/Solicitation Court:
    Consumers are on probation for duration of phases. Consumers receive medication management, skills training and case management services. Consumers are also on the non-TRR 1115 Waiver ANSA with 90-day Recovery Services Plan. CHCS treatment team meets with court team weekly for update on consumer dispositions. Upon successful completion of the program phases consumer can transfer to another ABH clinic or discharge to community services.


    Court Ordered Treatment - Not Guilty By Reason of Insanity (COT-NGRI): Consumers are on a court ordered treatment after having been found not guilty by reason of insanity.

    Consumers receive standard TRR services.

    Treatment team meets with court if called for testimony during renewal or modification processes and submits monthly and quarterly updates to court.

    Upon successful completion of court ordered commitment, consumer can transfer to another ABH clinic or discharge to community services.


    Outpatient Competency Restoration (OCR): Consumers are on court ordered treatment after having been found to be incompetent to stand trial.

    Receive standard TRR services for medication management, skills training/psychosocial rehab, and case management services.

    Treatment team meets with court if called for testimony during renewal or modification processes and submits monthly and quarterly updates to court.

    Primarily a mixture of LOC-1 and LOC-3. Upon successful completion of court ordered commitment, consumer can transfer to another ABH clinic or discharge to community services.


    ACCESS:


    This program provides outpatient mental health services, including medication management and psychosocial rehabilitation for individuals on Parole or Probation working with community supervision officers.

    Criminal Justice referrals needed.


    TCOOMMI- Genesis:
    Intensive outpatient program for felony probationers and parolees with serious and persistent mental illness (SPMI) and substance use challenges. This program is intended to divert individuals from the criminal justice system. Criminal Justice referrals only requiring weekly and monthly communication regarding treatment participation. This role is eligible for hybrid/remote work from home schedule.


    TCOOMMI- Diversion:
    Intensive outpatient program for individuals with serious and persistent mental illness (SPMI) on Pre-Trial supervision. Program individuals may be eligible to receive case management services, medication management, psychosocial rehabilitation, and substance abuse interventions. Criminal Justice referrals only requiring weekly and monthly communication regarding treatment participation. This role is eligible for hybrid/remote work from home schedule.


    DDRF TCOOMMI:


    Dual Diagnosis Residential Facility (DDRF) is a 120 to a 180-day residential treatment program serving adult male and female consumers diagnosed with mental health and substance use disorders.

    DDRF is a structured and focused jail diversion program aimed at reducing recidivism.

    The mission of the program is to equip and empower consumers to make positive, long-lasting changes that improve their quality of life and serves to positively impact the community.

    Judicial referrals only. DDRF is located on Applewhite Rd.


    Adult Enrollment Services Duties:

    • Assists with crisis intervention and resolution when needed.
    • Collaboratively develops initial recovery plans with consumer legal or legally authorized representative (LAR).
    • Cross trains to work with both adult and child population to help support community needs.
    • Uses current DSM criteria to render a diagnosis of mental illness/obtains diagnosis from collateral and or from licensed clinician along with other clinical assessment tools to determine level of care and medical necessity.

    Adult Services-Outpatient Duties:

    • Completes enrollments (psychosocial assessment and diagnostic assessment) for new/returning consumers into adult outpatient services.
    • Coordinates with multidisciplinary teams including care managers and medical professionals (to include both mental health and primary care providers).
    • Ensures clinical documentation is submitted in accordance with CHCS standards (within 23 hours of service delivery).
    • Maintaining needed licensure requirements and specialized certificates.
    • Provides direct clinical services and assessments and develops person-centered recovery plans for consumers.
    • Mixed caseloads (counseling, non-clinical, etc) may be assigned at the discretion of the unit leadership.
    • Performs other related duties as required.

    SMART Program Duties:

    • Provides crisis intervention in the SMART response model when dispatched to appropriate low level, 9-1-1, mental health related calls.
    • Participate in coordination and communication with BCSO and Acadian team members on the progress of individuals that are receiving services at the time of response and the linkage to the next level of support needed.
    • Provides individualized direct clinical services that may include: assessment and diagnostic impression, case coordination, case transfers, community referrals, counseling and psychotherapy, crisis services, discharge planning, medication training and support (individual and/or group), non-medical evaluation and management assessment, psychoeducation, psychosocial rehab services (individual and/or group), recovery planning, routine case management, skills training and development (individual and/or group), and other clinical interventions as assigned.
    • Complete and maintain required training, credentialing, licenses, and background checks.
    • Comply with and participate in Quality Assurance and Quality Improvement processes.
    • Adhere to established CHCS Employee Handbook and policies and procedures and Bexar County response procedures.
    • Documents all clinical contacts in Anasazi or EHR system.
    • Performs other related duties as required.

    MINIMUM ENTRANCE QUALIFICATIONS
    Education and Experience


    • Graduate Degree in Social Work, Psychology, Counseling, or a related behavioral health field
    • Complete the Qualified Mental Health Professional Community Services (QMHP-CS) within six (6) weeks of hire or transfer
    Licenses or Certifications


    • Must have and maintain current license in good standing in the State of Texas as a Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), and/or Licensed Psychologist (LPHD)
    • Must be able to be credentialed and enrolled with CHCS payors (determined by license type and subject to Provider regulations)
    Other Requirements


    • Must maintain a valid driver's license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Company's auto insurance carrier if you drive your vehicle during company business.
    • Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies.
    • Must be able to meet the physical requirements to complete SAMA and CPR training including lifting up to 12 lbs. and supporting up to 55 lbs.; bending, stooping and getting on and off the floor without assistance.
    • Justice Intake & Assessment Annex (JIAA)

    Programs:

    Licensed Clinicians working within this program must be able to lift at least 50 lbs to maneuver boxes of completed assessments.


    • Depending on assigned unit/program, applicants for this position must pass a Criminal Justice Information Systems (CJIS) fingerprint-based background check and maintain CJIS eligibility.
    Due to CJIS requirements related to system access, the following may result in being disqualified for this position:

    Felony Convictions, Felony Deferred Adjudication, Class A & B Misdemeanor Deferred Adjudication, Class B Misdemeanor Convictions, an Open Arrest for Any Criminal Offense (Felony or Misdemeanor), and Family Violence Convictions.


    PREFERRED QUALIFICATIONS

    • Three (3) years of progressively responsible experience working with individuals with substance use and/or mental illness.
    • Prior experience with or knowledge of trauma-informed care.
    • Bilingual (English/Spanish) preferred, subject to 50% Spanish-speaking consumer caseload and internal testing.

    SUPERVISION

    • Job has no responsibility for the direction or supervision of others.
