    Job Title: Licensed Practical Nurse (LPN)

    We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Trinity Hill Care.

    About the Role

    This is an excellent opportunity to provide direct care to residents in our skilled nursing facility, working in a fast-paced and dynamic environment. As an LPN, you will be responsible for delivering high-quality patient care, collaborating with healthcare professionals, and contributing to the overall well-being of our residents.

    Responsibilities

    • Provide direct patient care, including administering medications, monitoring vital signs, and documenting patient information
    • Collaborate with interdisciplinary teams to develop and implement individualized care plans
    • Participate in quality improvement initiatives to ensure exceptional patient outcomes
    • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions

    Requirements

    • State of Connecticut nursing license, required
    • Proficiency in English, required
    • Previous LPN experience is preferred, but not required
    • Covid-19 Primary Series vaccination required
