    Summary



    This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until April 11, 2024, or until vacancy is filled. The first group of eligible applicants will be reviewed January 25, 2024. Subsequent reviews will be conducted as needed until position is filled.



    Duties


    Major Duties include, but are not limited to:

    • Demonstrate clinical competencies in such areas as age-specific care, cultural assessment, and health

      education
    • Function autonomously, as well as a member of an interdisciplinary team
    • Provide organized and safe patient care
    • Focus on individual assignment to address concerns and issues occurring with assignments
    • Manage multiple competing priorities safely
    • Assess care provided and seeks to improve
    • Accurate and timely documentation of patient care according to the standards of the work area
    • Demonstrate understanding of and compliance with patient safety guidelines, accrediting body guidelines, VHA Directives, and Facility Center Memorandums that pertain to the work unit and direct patient care
    • Demonstrate understanding of facility performance metrics that can be influenced by care provided
    • Active participation in program level quality improvement processes and initiatives
    • Use of metrics to show improvement in delivery of care
    • Active participation in customer service improvement processes and initiatives
    • Maintain current (not expired) licensure, certifications (BLS/ACLS), competencies, and mandatory trainings
    • Notify the Nurse Manager of any safety incidents, employee injuries, or concerns of team members not providing care according to the appropriate Standards of Care
    • Attend staff meetings and program specific education
    • Follow and include the appropriate Nursing chain of command structure
    • Commitment to discipline-specific standards of practice, including: ethical conduct, the primary care delivery model, chronic disease management, palliative care, principles of care management, a holistic framework of practice, and geriatric care skills (including infection control, equipment and environmental safety)
    • Assist in the implementation of a geri-palli care quality improvement plan for facility, and report findings regularly to the nurse manager and applicable leadership with recommended action plans
    • Educate the patient and the caregiver as needed
    • Coordinate the process of care with the other team members to maximize efficiency
    • Assess the patient's continuing need for Geri-Palliative care, and plan for discharge when indicated
    • Participate in Geri-Palliative team meetings
    • Geri-Monitor and provide applicable follow-up to ensure proper utilization of healthcare resources to ensure workload capture
    • Geri-Aware of program quality improvement plans
    • Point of contact and resource for geriatric, hospice and palliative care issues
    • Collaborate with various disciplines such as social work, psychology, specialty clinics, caregiver support, Home Based Primary Care, to ensure that Veterans get the care they deserve
    • Consider all aspects of the Veteran, including age and life stage, state of health, race, ethnicity, and culture, values, socioeconomic status and previous experience when providing care
