    The mission of the Transitional Care Coordination Program is to ensure intensive, coordinated, outpatient care for patients with complex chronic health problems that leave them medically vulnerable.

    to improve health and well-being through coordinated medical services with emphasis on the social determinants that affect health such as housing, food and transportation.

    We do this by providing coordination and support for complex, vulnerable patients whose chronic health conditions and psychosocial challenges place them at high risk for repeated hospital admission or frequent returns to the emergency room.

    We provide comprehensive post hospital care coordination via telephone outreach, community visits and in-home visits as needed to ensure patient is supported in their health care needs/goals after they return to the community.

    Appropriate and affordable medical care (FQHC, immigrant services, primary & secondary care, utilizing our high risk model bridge clinic (the Coordinated Care Center) for those who are medically complex and require services that are of a higher intensity, longer duration or higher frequency visits than can be met in the community)
    Social programs (food resources, supplemental income)
    Services to access care (ID, insurance)
    Transportation (mobility, medical assistance transportation, bus services)
    Housing (shelters, affordable housing resources, assisted living)
    Mental health services

    nIndependently practices social work and receives limited consultation , functioning as a member of the multidisciplinary healthcare team to provide case management, therapeutic crisis intervention, biopsychosocial assessment, clinical evaluation, therapy, counseling, referral, treatment planning, and other clinical social work services within their practice area.

    Offers psychosocial guidance, with their advanced ability, to help settings enhance their clinical practice. Drives conversations about Advance Care Planning to healthcare team.
    # Provides advanced ongoing and crisis care to facilitate a therapeutic and safe environment for patients. Serves as a professional resource to other members of care team.
    # Suggests/initiates PI projects to evaluate program and outcome effectiveness.
    # Demonstrates advanced knowledge and skillsets necessary to provide care appropriate to the age, culture, ethnicity, spirituality, and individual diagnostic needs of the persons served.
    # Demonstrates advanced knowledge of relevant resources to share with team members & initiates networking based on program gaps. Empowers patients and the practice area with problem-solving and decision-making around personal, health, social, educational, vocational, financial and other interpersonal concerns within context of discharge planning, clinic visit, or on-going behavioral health assistance.
    # Provides consultation and assistance with challenging cases to other social workers.
    # Conducts clinical reviews of assessments, planning, interventions, and overall plan of care to provide peer feedback.
    # Participates in conversations about unit practice & provides recommendations for improvement.
    # Master's degree in Social Work accredited by Council on Social Work Education (CSWE).
    # LCSW- C (Licensed Certified Social Worker-Clinical) licensure from the Maryland Board of Social Work Examiners.
    # Demonstrates skill in a) clinical social work and/or case management practice areas; b) completing clinical social work functions such as clinical assessment, treatment planning, clinical intervention, case management, psychoeducation, referral, psychotherapy, counseling, diagnosis of mental health disorders, advocacy; and e) age appropriate interpersonal interactions (patients may range from newborn to geriatric adult).
