- Utilize EMR and PPS software to monitor patients' progression along the care pathway
- Collaborate with Providers to set up efficient Patient and Clinical Workflows
- Develop strong, trusting relationships with patients
- Adhere to the Clinical Care Pathway by reaching out to patients at specified checkpoints in their healthcare journey
- Educate patients on care instructions and ensure they commit to follow-up appointments (and prepare them for the appointments)
- Address any queries patients may have (requiring potential follow-up communications)
- Demonstrate problem-solving and analytical abilities to overcome patient-related challenges
- Maintain thorough and precise documentation in patient records and PPS Software
- Report any procedural or care pathway issues promptly to the Manager
- Comprehend and uphold HIPAA regulations and the significance of safeguarding patient information
- Availability required at the Radiation Center once a week, with the rest of the week based in the office
- Perform any other duties as delegated
- Experienced Medical Assistant or Licensed Vocational Nurse
