    Position Overview:

    We are looking for a dedicated Patient Navigator specializing in Prostate Cancer to ensure that patients with prostate cancer follow the established care pathway back to the clinical practice. The key objective of this role is to guide patients in discovering the most suitable treatment options for their individual needs.


    Roles and Responsibilities:

    • Utilize EMR and PPS software to monitor patients' progression along the care pathway
    • Collaborate with Providers to set up efficient Patient and Clinical Workflows
    • Develop strong, trusting relationships with patients
    • Adhere to the Clinical Care Pathway by reaching out to patients at specified checkpoints in their healthcare journey
    • Educate patients on care instructions and ensure they commit to follow-up appointments (and prepare them for the appointments)
    • Address any queries patients may have (requiring potential follow-up communications)
    • Demonstrate problem-solving and analytical abilities to overcome patient-related challenges
    • Maintain thorough and precise documentation in patient records and PPS Software
    • Report any procedural or care pathway issues promptly to the Manager
    • Comprehend and uphold HIPAA regulations and the significance of safeguarding patient information
    • Availability required at the Radiation Center once a week, with the rest of the week based in the office
    • Perform any other duties as delegated

    Requirements and Skills:

    • Experienced Medical Assistant or Licensed Vocational Nurse
    • Proficiency in prostate cancer disease states and treatments
