    Job Description: Medical Liaison - Licensed Practical Nurse

    Primary Work Location: Harc, Inc. Day Program, 900 Asylum Ave. Hartford, CT

    Primary Work Schedule: Monday - Friday, daytime hours

    I.OCCUPATIONAL DESCRIPTION

    1. Assist Program Registered Nurse (RN) and Director of Nursing (DON) in the following areas:

    a. Provide oversight and maintain records pertaining to the DDS Medication Administration Certification for new and recertifying staff.

    b. Delegate routine and/or specific responsibilities to Med Cert Staff.

    c. Monitor overall compliance with DDS agency policies and procedures.

    d. Train support staff in areas that pertain to the individuals supported by Harc, Inc. including, but not limited to, medication administration, nutrition by G/tube, universal precautions, medication precautions and all associated documentation requirements.

    e. Conduct other DDS required staff training as designated by the Program RN and/or DON.

    2. Support the individuals by assisting them with the following:

    a. Monitor the overall health of supported individuals including, signs and symptoms of chronic and acute medical conditions.

    b. Ensure accurate transcription of doctor's orders and communicating directly with the pharmacy.

    c. Monitor medication, medication MAR's/Kardex daily, medication errors and MD orders.

    d. Communicating current medical information to the Program RN and/or DON, medical providers, staff, and families.

    e. Observe medication certified staff during medication passes.

    f. Review monthly kardexes before the next month and make any necessary changes. Assist Program RN and/or DON in maintaining quarterly med audits and quarterly nursing reviews when necessary.

    g. Document all relevant information in DDNC program-focus notes.

    h. Keep in close communication with the Program RN and/or DON i.e. all medication changes, changes in health, med errors etc.

    i. Complete monthly Med Room Closet Audits including the destruction of expired medications with Program RN/DON supervision, in compliance with relevant legal requirements and the organization's policies and procedures.

    j. Provide direct support coverage to individuals in lieu of staff or when otherwise deemed necessary by the Program RN and/or DON, including but not limited to, attending medical appointments (residential only), administering medications, providing personal care, etc.

    3. Complete and maintain current all training required through the new employee orientation process including CPR, Bloodborne Pathogens and any other ongoing training that may be required to fully support the residents.

    4. Successful completion of the DDS Training Course for LPN's.

    5. Monitor, report and document circumstances occurring at the time a person experiences a seizure, injury, illness, behavioral episode, and/or any other medical or emotional event. This information will be reported to the Program RN, Director of Nursing and appropriate program management staff.

    6. Support the individuals by being an active participant during Planning and Support Team Meetings. Participation includes, but is not limited to; attending team meetings, helping to assure medical progress data is being reported/recorded or obtained on a consistent basis.

    7. Provide support and guidance to the individuals so they may have success integrating into the social, emotional and physical situations of daily living.

    8. Provide assistance to staff to ensure that individual dietary requirements are met and guidelines are followed.

    9. Support individuals by assisting them with personal hygiene needs such as, tooth brushing, flossing, showering, toileting, menstruation care. These supports will range from intermittent to pervasive levels.

    10. Attend meetings when necessary as determined by Program RN, Director of Nursing, and/or Harc, Inc. administration.

    II.REQUIREMENTS: EDUCATION, TRAINING AND EXPERIENCE

    1. Graduation from an accredited LPN school. Must exhibit a basic knowledge of current nursing practices and hold a current Connecticut LPN license. One year's experience working with people with developmental disability.

    2. Has strong organizational and time management skills.

    3. Demonstrates the ability to communicate to persons with developmental disability using multi-modality communication systems.

    4. Has the ability to guide and serve as a mentor to other staff in Harc, Inc. programs.

    5. Has a working knowledge of positive educational approaches and reinforcement programs that will support each resident's individual strengths and adaptive limitations.

    6. Demonstrates knowledge and interest in current issues that affect the individual's daily needs.

    7. Must possess a valid driver's license and maintain a liability free driving record at all times to help ensure residents' safety.

    8. Has the ability to maintain consistent professional ethics as they relate to the organization's mission.

    9. Physical demands: Work is active; may vary from light to heavy as defined by the Dictionary of Occupational Titles (1991 Edition). It is performed in Harc, Inc. programs. A variety of lifting and transfer supports for the individuals may be required to facilitate full independence.

    III.WORKER TRAITS

    Aptitude – Must possess strong verbal and written communication skills and a high level of professionalism.

    Proficient computer skills including Microsoft Word, Excel, and ability learn the DDNCS medical record system.

    To communicate with people who require a variety of support levels, i.e., from intermittent to pervasive supports, to demonstrate strong organizational abilities, implement programs, services and perform other tasks required to enhance the lives of supported individuals; to work as a team player with all other support staff with the goal of providing any and all needed levels of support to the individuals.
