    Job Title:
    After-Hours Registered Nurse (RN) – On-Call Visiting Nurse

    Schedule:
    4:30pm – 8:00am

    Overview

    We are hiring an After-Hours RN to provide phone triage and in-home nursing visits as needed.

    This role is primarily remote, responding to participant and family calls, assessing needs, and determining appropriate interventions including home visits, provider communication, or higher levels of care.

    Responsibilities

    Provide on-call triage and clinical assessment
    Perform home visits when medically necessary
    Coordinate care with providers and interdisciplinary teams
    Support end-of-life care and family education
    Maintain accurate EMR documentation
    Participate in team handoffs and care plan reviews
    Requirements

    Active RN license
    BLS certification (AHA or American Red Cross)
    2+ years healthcare experience (geriatrics preferred)
    1+ year home health experience
    Associate's Degree in Nursing
    Valid driver's license and reliable transportation
    Preferred


    BSN
    Hospice or end-of-life experience
    Bilingual candidates encouraged to apply
