Description
Join a thriving rural practice providing a full spectrum of care in Central Minnesota~ Join a team of 6 FM physicians and 4 APPs
~4-day work week, 3 days in clinic and 1 day in the Level 4 Emergency Department
~ Additional ED shifts available for those interested.
Weekdays call is approximately 1:6, providing backup call to APP overnights.
Weekend call with primary hospitalist duties including admissions and rounding.
CentraCare - Melrose Clinic
Dedicated primary clinic nurse (CMA, LPN or RN) with support of float nursing
Co-located workstations for provider/nursing with an additional office
Opportunity for voice-recognition software (Dragon) or scribe
Team support including RN nurse lead visits, diabetic education, health care home for complex patient care, legal aide, health care navigator for uninsured/underinsured
Community health worker and community paramedic for patient engagement and home visits
CentraCare Foundation supported Reach Out & Read Program
Mental Health support including Integrated Behavioral Health with often same-day video consultation and Psychiatry chart review for medication management

State-of-the-art Critical Access Hospital built in 2017 with significant provider and community input
Cloud Hospital
Advanced imaging with 5 days/week MRI availability, 3D Mammography
Transitional Care Team including a nursing supervisor, PT, OT, speech therapy and social worker
