    Description

    A nonprofit organization in Brooklyn is actively seeking a compassionate and dedicated Case Manager to join their Permanent Housing team. In this role, the Case Manager will be responsible for providing support services to help individuals remain housed and to address needs such as benefits/entitlements, activities of daily living, substance abuse, and mental health services.

    Responsibilities

    The Case Manager will:

    • Provide direct services to clients
    • Conduct initial and ongoing assessments of clients
    • Connect clients to medical treatment and substance use treatment facilities when applicable
    • Meet weekly with clients to assess needs, encourage harm reduction, and develop service plans
    • Conduct psychosocial evaluations
    • Connect clients to psychiatric services
    • Obtain identification, public benefits, and other applicable income sources for clients
    • Connect clients to job training programs, recreational activities, community services, food and nutrition programs
    • Assist clients with preparation of documents needed for recertification and maintaining their housing
    • Escort clients to appointments when needed
    • Intervene in crisis situations
    • Perform other related duties as assigned

    Qualifications

    • Bachelor's degree
    • 2 years work experience
    • Excellent computer skills, including proficiency in Microsoft Word and Excel and the ability to learn new programs
    • Strong attention to detail
    • Ability to multitask
    • Demonstrated interpersonal skills
    • Excellent writing and communication skills

    Desired skills

    • Bilingual, English and Spanish
    • 2 years of experience working with homeless adults and substance abuse issues
    • Experience with harm reduction and motivational interviewing techniques
