- Provide direct services to clients
- Conduct initial and ongoing assessments of clients
- Connect clients to medical treatment and substance use treatment facilities when applicable
- Meet weekly with clients to assess needs, encourage harm reduction, and develop service plans
- Conduct psychosocial evaluations
- Connect clients to psychiatric services
- Obtain identification, public benefits, and other applicable income sources for clients
- Connect clients to job training programs, recreational activities, community services, food and nutrition programs
- Assist clients with preparation of documents needed for recertification and maintaining their housing
- Escort clients to appointments when needed
- Intervene in crisis situations
- Perform other related duties as assigned
- Bachelor's degree
- 2 years work experience
- Excellent computer skills, including proficiency in Microsoft Word and Excel and the ability to learn new programs
- Strong attention to detail
- Ability to multitask
- Demonstrated interpersonal skills
- Excellent writing and communication skills
- Bilingual, English and Spanish
- 2 years of experience working with homeless adults and substance abuse issues
- Experience with harm reduction and motivational interviewing techniques
