Job summary
We are seeking a Health Coordinator to join our team at UnitedHealthcare. In this role,you will work in the field and coordinate the long-term care needs for patients in the local community.You will assess plan implement care strategies individualized by patient directed toward most appropriate least restrictive level of care identify initiate referrals social service programs including financial psychosocial community state supportive services manage care plan throughout continuum of care single point contact communicate stakeholders required health related information ensure quality coordinated care services provided expeditiously members advocate patients families needed patient needs choices represented supported healthcare team
You must be able to travel assigned region visit Medicaid members homes other settings including community centers hospitals providers offices have current access ability obtain internet access landline driver license reliable transportation
Responsibilities
- Assess plan implement care strategies individualized by patient directed toward most appropriate least restrictive level of care
- Identify initiate referrals social service programs including financial psychosocial community state supportive services
- Manage care plan throughout continuum of care single point contact
